Obesity<\/strong> is a chronic, progressive, and relapsing disease. In Spain, one in six people suffers from obesity, and in the US, one in three. It’s estimated that by 2035, 37% of the Spanish population will be obese. In 2023, for the first time in US history, the obese population decreased by 1.6% thanks to new medications.<\/p>\n\n\n\n

Obesity is a multifactorial disease; there are genetic, hormonal, metabolic, environmental, dietary, and lifestyle factors involved.<\/p>\n\n\n\n

What are the benefits of weight loss?<\/h3>\n\n\n\n

Losing 5-10% of body mass helps prevent type II diabetes, improve polycystic ovary syndrome<\/strong><\/a>, lipid abnormalities, and the incidence of fatty liver disease.<\/p>\n\n\n\n

If the loss is 10-15%, it improves cardiovascular disease, stress urinary incontinence, knee osteoarthritis, sleep apnea, fatty liver disease, and gastroesophageal reflux.<\/p>\n\n\n\n

And with a loss of over 25%, type II diabetes remission is achieved, heart failure improves, and death from cardiovascular disease and other causes decreases.<\/p>\n\n\n\n

Regarding assisted reproduction and fertility<\/a><\/strong>, weight loss also improves the chances of spontaneous pregnancy, decreases the incidence of miscarriage<\/a><\/strong>, and leads to better success rates<\/strong><\/a> following assisted reproduction techniques.<\/p>\n\n\n\n

These medications act on incretins, intestinal peptides released in the duodenum and jejunum (GIP), and in the ileum and colon (GLP-1). These peptides are released upon glucose intake, have a very short half-life of 3-6 minutes, and act on the pancreas, promoting insulin release and decreasing glucagon levels. The new medications are able to extend the duration of incretin action, which has effects throughout the body. At the central nervous system level, they increase feelings of fullness and promote lower food intake; at the liver level, they reduce fat formation; at the intestinal level, they slow transit and gastric emptying. In white fat, they promote lipolysis (fat breakdown), and in brown fat, they promote thermogenesis. All these effects contribute to weight reduction.<\/p>\n\n\n\n

What medications are currently available?<\/h3>\n\n\n\n

Six years ago, Liraglutide (Saxenda) began to be marketed in Spain. It’s a GLP-1 incretin agonist that promotes insulin secretion and reduces blood glucagon levels. It achieves an average weight loss of around 8.4 kg, and improves blood glucose and prediabetes. Side effects are mostly gastrointestinal: nausea, vomiting, diarrhea or constipation, gallstones, and cholecystitis, as a consequence of long fasting periods. A 2% incidence of mild drug-induced pancreatitis has been reported. Due to these side effects, the discontinuation rate is 10%.<\/p>\n\n\n\n

Later, Semaglutide (Ozempic, Wegovy) appeared, a GLP-1 incretin agonist that achieves more pronounced weight loss than Liraglutide. 54% of patients lose more than 15% of their body weight, and 34% lose more than 20%. It has fewer side effects and, therefore, a lower discontinuation rate (1).<\/p>\n\n\n\n

Currently, in Spain we have Tirzepatide (Mounjaro) available, a GLP-1 and GIP agonist. At the pancreatic level, it promotes insulin secretion; in fatty tissue, fat breakdown; and at the brain level, it reduces appetite. Side effects are milder than with Semaglutide. In a clinical trial (2) published in 2022 involving 2,500 patients, with doses of 5, 10, and 15 mg, it was observed that the lowest dose achieved 15% weight loss, and with the 15 mg dose, 50% of patients had weight loss of over 20%, with more than 30% achieving weight loss of over 25%. This comes very close to the results obtained through bariatric surgery, which achieves 30% weight loss. In 2024, a comparative trial between Semaglutide and Tirzepatide (3) showed average weight loss of 15-17% with the former and 25.8% with the latter.<\/p>\n\n\n\n

These are very recent medications; the first ones appeared in the year 2000. We know that after stopping them, weight is gradually regained, and for some patients therapy may need to be long-term. It’s not yet defined whether discontinuation should be abrupt or gradual; however, a gradual approach is currently recommended. A multidisciplinary approach is essential, including lifestyle changes, diet, exercise, and support from various professionals such as endocrinologists, psychologists, and nutritionists.<\/p>\n\n\n\n

Their use is contraindicated during breastfeeding and pregnancy. It’s recommended to stop therapy 8 weeks before pregnancy in the case of Semaglutide, and 4 weeks before in the case of Tirzepatide. Tirzepatide may reduce the absorption of oral contraceptives, and if pregnancy isn’t desired, additional contraceptive measures should be used during the first 4 weeks of use. This isn’t necessary with Semaglutide.<\/p>\n\n\n\n

Another important consideration is that both medications slow gastric emptying, and if anesthesia is required, their use should be stopped 7 days beforehand due to the risk of aspiration.<\/p>\n\n\n\n

Their use in infertile patients is becoming increasingly common, since we know that obesity has a negative impact on both egg quality and implantation. Chronically elevated inflammatory factors in these patients negatively affect their fertility and are also linked to a higher incidence of miscarriage. In many cases, waiting 3 to 6 months before starting assisted reproduction treatment, with the goal of achieving at least 10% weight loss, will have a very positive impact on outcomes.<\/p>\n\n\n\n

Other new medications are being tested and will be available soon. These are very powerful drugs that can change people’s lives, allowing them to live longer and better.<\/p>\n\n\n\n
