What is egg donation and when is it recommended?

Egg donation consists of using eggs from a young, healthy donor which are fertilised in the laboratory so that the embryo can be transferred to the recipient woman’s uterus.

It is recommended in cases of:

  • low ovarian reserve
  • menopause or premature ovarian failure
  • previous IVF failures
  • genetic risk

What is the egg donation treatment like?

Egg donation in our country is entirely legal; the law states that it must be anonymous and voluntary.

Clínica Fertia donors undergo exhaustive medical checks which include interviews and medical tests (examination, blood tests, ultrasound scans, etc.) in order to verify their physical health and ensure that they are not carriers of hereditary conditions.

Clínica Fertia has a large number of donors, which makes it easy to find donors with physical features similar to those of the recipient. The usual waiting time is approximately two or three weeks, although it may be longer, especially in the case of recipients of non-Caucasian ethnicity.

Donors are exclusive, that is, each donor donates her eggs to a single recipient, with an average number of oocytes obtained of 10-12.

Once we have selected the most suitable donor, the synchronisation of the donor’s and the recipient’s cycles is carried out. We make sure that ovarian activity in the recipient is stopped, for which we usually use contraceptives or a single dose of GnRH analogues, except in women who are already menopausal, in which case it is not necessary. Next, a replacement regimen with oestrogens is started, either orally or through transdermal patches, to ensure adequate preparation of the endometrium, where the embryos will be placed. During this process one or two ultrasound scans will be carried out to check that the endometrium has reached the desired thickness. This phase usually lasts around two weeks.

In a natural cycle a woman produces only one egg. The same hormones that control the natural cycle are administered to the donor in higher doses, with the aim of obtaining an average of 10-12 oocytes. This process lasts around 10-12 days. Throughout this phase ultrasound checks are carried out to monitor the number and size of the developing follicles, while adequate endometrial development is checked in the recipient.

Once the development of the follicles is adequate, a final dose of hormones is administered to complete the maturation of the eggs, and their retrieval is scheduled for 36 hours later.

It consists of the aspiration of the oocytes guided by transvaginal ultrasound. Once carried out, the recipient is informed of the number of oocytes obtained, as well as the instructions for starting progesterone treatment.

Once the eggs have been obtained, they are inseminated with the semen of her partner, previously prepared in the laboratory. Sperm selection can be carried out using a technique known as MACS, immunomagnetic selection of the sperm with the greatest capacity to fertilise the egg, or by conventional capacitation methods.

For fertilisation, a technique known as Intracytoplasmic Sperm Injection, ICSI is used. It consists of introducing into the egg a single live, previously selected sperm; thanks to this, fertilisation can be carried out in cases of poor sperm quality.

In those cases where it is necessary, donor semen may be used.

TESE can also be applied; it consists of performing ICSI with sperm obtained by testicular biopsy, in cases of obstructive azoospermia or when the sperm suffer severe damage during the process of leaving the testicle.

The number of fertilised oocytes is checked the following day; the biologists will report daily on their development up to the day of the transfer.

 

It consists of the introduction of the embryos inside the uterine cavity. To do this, a speculum is placed in the vagina and a fine cannula carrying the embryos is introduced; the embryos are deposited in the uterus under ultrasound guidance. This process is quick and painless.

The transfer is carried out two to six days after the egg retrieval, although it usually takes place five days later. A recommendation is usually given on the number of embryos to transfer, but the final decision always rests with the couple.

After the transfer we recommend relative rest that day and the following day, and then a return to your usual activity. We only recommend avoiding strenuous effort and sport, and not having sexual intercourse, for the following two weeks. Travelling poses no problem.

It is a process of cellular solidification at low temperatures (-196ºC) which are reached very rapidly, preventing the formation of crystals. Once vitrified, the eggs are stored inside straws using a closed system that prevents them from coming into contact with the liquid nitrogen in which they are immersed for preservation, thus guaranteeing the highest safety conditions.

This can be done by measuring BHCG in the blood 10 days after the embryo transfer.

Or by taking a urine test 14 days after the transfer.

Until confirmation, the medication prescribed on the day of the transfer must be continued.

Fifteen days after the BHCG test, an ultrasound scan is carried out to confirm that there is a gestational sac; follow-up will continue at the clinic until a heartbeat is observed. It is recommended to continue hormone replacement treatment at least until week 10 of pregnancy.
