What is in vitro fertilisation and when is it recommended?
In vitro fertilisation is an assisted reproduction treatment that involves fertilising the egg with sperm in the laboratory and transferring the embryo to the uterus.
It is recommended in cases such as:
- women aged 38 or over

What is the in vitro fertilisation treatment like?
The entire process usually takes between 3 and 5 weeks, depending on the case.
Normally, a single egg is produced in each natural cycle. Ovarian stimulation involves administering hormones via subcutaneous injection to obtain more eggs. The entire process is monitored using ultrasound scans, for which we are available seven days a week. Depending on the type of protocol used and how quickly the ovaries respond, the process will take between 10 and 12 days.
Once the follicles have reached the appropriate number and size, a hormone called HCG is administered to complete the maturation of the eggs. Approximately 36 hours after administering the HCG, the mature eggs can be retrieved from the ovary.
This involves the aspiration of the mature oocytes under ultrasound guidance and with mild sedation, so that the patient does not experience any discomfort. The procedure takes around 15 minutes. On that day it is recommended that you avoid activities requiring your full attention, such as driving, even if you feel perfectly well.
Once the eggs have been retrieved, they are inseminated with semen that has been previously processed in the laboratory. Sperm selection can be carried out using a technique known as MACS or using conventional capacitation methods such as concentration gradients, Swim Up…
Fertilisation can be carried out using IVF or ICSI. In IVF, the biologist brings each oocyte into contact with the previously selected sperm and allows fertilisation to take place naturally. In contrast, in ICSI, a single sperm is injected into the egg using a very fine needle, approximately 15 microns in diameter. ICSI is mainly used in cases of severe sperm abnormalities, poor oocyte quality, valuable or scarce eggs, advanced maternal age, suspected low fertilisation rates, and cases of previous failure with conventional IVF.
When there are no sperm in the ejaculate (azoospermia) but sperm are present in the testicle, a testicular biopsy (TESE) may be performed. In this case, the sperm are retrieved from testicular tissue and cultured in the laboratory beforehand. The technique used in these cases will always be ICSI.
The embryos are cultured and monitored in the laboratory for a few days (2 to 6). To do this, we use modern incubation techniques to monitor their development in real time (morphokinetics). We have incubators equipped with time lapse technology, which incorporates an image capture system, allowing us to observe the embryos from the moment of fertilisation until they are transferred to the uterus, without having to remove them from the incubator. This ensures more stable culture conditions, which leads to improved embryo quality. Furthermore, this technology facilitates the selection of the best embryos for transfer and freezing.
It involves the introduction of the embryos into the uterine cavity. To do this, a speculum is inserted into the vagina and a fine cannula carrying the embryos is introduced; the embryos are then deposited into the uterus under ultrasound guidance. This process is quick and painless.
Following the transfer, we recommend relative rest on the day of the procedure and the following day, after which you may resume your normal activities. You are advised to avoid strenuous exertion and sport for the following two weeks. Travelling poses no problem.
This is a process of cellular solidification at low temperatures (-196ºC), which are reached very rapidly, thereby preventing the formation of crystals. Once vitrified, the eggs are stored inside straws using a closed system that prevents them from coming into contact with the liquid nitrogen in which they are immersed for preservation, thus ensuring the highest safety standards.
This can be done by measuring BHCG levels in a blood test 10 days after the embryo transfer.
Alternatively, a urine test can be carried out 14 days after the transfer.
The medication prescribed on the day of the transfer must be continued until confirmation.
Fifteen days after the BHCG test, an ultrasound scan is carried out to confirm embryo implantation; the gestational sac containing the embryo will be visible, along with a heartbeat. On some occasions, the heartbeat may not be visible until 7 days later.
